Medical Disclaimer Services provided by Empowered Medical Spa are intended for aesthetic and wellness purposes only and are not intended to diagnose, treat, cure, or prevent any disease or medical condition unless specifically stated by a licensed medical provider. Individual results may vary. No guarantees are made regarding treatment outcomes. Clients are responsible for disclosing accurate and complete medical history, medications, allergies, and other relevant health information prior to receiving treatment. Appointments & Cancellations Appointments may require a valid credit or debit card to reserve. By scheduling an appointment, you agree to our cancellation policy: Appointments canceled or rescheduled with at least 24 hours’ notice will not incur a fee. Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee. No-show appointments may be charged up to the full service amount. Late arrivals may result in shortened appointment times or rescheduling at management discretion. Payments Payment is due at the time services are rendered unless otherwise agreed in writing. We accept major credit cards, debit cards, and other approved payment methods. By providing payment information, you represent that you are authorized to use the payment method submitted. Refund Policy All sales are final unless otherwise required by law. Consent to Treatment Certain services require signed informed consent documents prior to treatment. Chargebacks & Billing Disputes Clients agree to contact Empowered Medical Spa directly to attempt resolution before filing a chargeback or payment dispute with a financial institution. We reserve the right to provide signed agreements, intake forms, treatment records, communications, and proof of services rendered in response to disputed transactions. Governing Law These Terms & Conditions shall be governed by and interpreted in accordance with the laws of the State of Michigan, without regard to conflict of law principles.
