Membership Overview
The Empowered Weight Management Membership provides ongoing provider oversight, treatment planning, refill evaluations, progress monitoring, and support for active weight management patients.
An active membership is required for all weight management services unless otherwise specified within an eligible maintenance care plan.
Membership fees do not include medication costs.
Membership Fee
Membership is billed every 4 weeks at the current published rate.
Membership fees are separate from treatment, medication, laboratory, imaging, or pharmacy costs.
Membership pricing is subject to change with advance notice.

Services may be modified as the program evolves.
Check-In Requirements
Patients are responsible for completing required Check-In/Refill Request forms approximately every 4 weeks while actively receiving treatment.
Check-ins should be submitted 1–2 weeks before medication is needed to allow adequate time for provider review and processing.
Failure to complete required check-ins may result in delays in treatment or refill requests.
Refill Requests
All refill requests require provider review.
Submission of a refill request does not guarantee approval.
Additional information, laboratory testing, appointments, or treatment modifications may be required before treatment can continue.
Treatment Eligibility
Treatment recommendations are based on clinical judgment, safety considerations, patient goals, medical history, and response to treatment.
Empowered reserves the right to decline, delay, modify, or discontinue treatment when clinically appropriate.
Medication Costs
Medication costs vary based on:

Patients are responsible for reviewing current treatment pricing before purchasing treatment.
Pharmacy Selection
Depending on the treatment plan and medication selected, prescriptions may be coordinated through Empowered pharmacy partners or sent to a pharmacy of the patient's choice when appropriate.
Availability, pricing, and formulations may vary between pharmacies.
Maintenance Care Plans
Maintenance care plans are available only to eligible patients on stable doses who do not require frequent treatment adjustments.
Eligibility is determined by provider review.
Maintenance plans include membership throughout the selected plan period.
If additional medication is required due to dose changes or treatment modifications, additional treatment costs may apply.
Cancellation
Membership may be canceled at any time.
Cancellation requests must be submitted prior to the next billing cycle.
Cancellation of membership may affect eligibility for continued treatment, refill requests, and provider oversight.
Previously billed membership fees are non-refundable.
Refund Policy
Membership fees are non-refundable once billed.
Provider review fees, treatment costs, medications dispensed, prescriptions already issued, and services already rendered are non-refundable.
Communication
Patients consent to communication through secure electronic methods including email, text message, patient portal messaging, and other approved communication platforms.
Patients are responsible for maintaining current contact information.
Changes to Terms
Empowered reserves the right to modify these Terms & Conditions at any time.
Updated terms will be posted on this page and become effective upon publication.
Continued participation in the program constitutes acceptance of updated terms.

By enrolling in the Empowered Weight Management Membership Program, I acknowledge that I have read, understood, and agree to these Terms & Conditions.
